Pentagon launches mandatory testosterone screening for troops over 30

Defense Secretary Pete Hegseth announced a new program requiring annual testosterone screening for all U.S. service members over 30, with testosterone replacement therapy available to those diagnosed as deficient. Hegseth framed the initiative as a combat-readiness measure, not a performance-enhancement scheme, and Democrats immediately tried to turn it into a culture-war talking point.

Hegseth posted a video to X on Wednesday under the headline “The High-T Department of War,” laying out the program’s rationale in direct terms. Troops aged 30 and older will undergo testosterone testing as part of their existing periodic  health assessments. Service members under 30 can volunteer for the screening. Treatment, for those who qualify, remains entirely optional.

The policy sits at the intersection of military medicine and  political theater. On the substance, testosterone deficiency affects roughly 5.6 percent of men between 30 and 79 and can cause muscle loss, fatigue, weight gain, depression, cardiovascular disease, and osteoporosis, as The Hill has reported. On the politics, the announcement drew instant fire from congressional Democrats who compared it, without apparent irony, to “gender-affirming care.”

Hegseth’s case: warfighter health, not enhancement

Hegseth built his argument around a simple premise: the Pentagon invests billions in hardware but has neglected the biological foundation of the people who operate it.

“While we invest heavily in our weapon systems, platforms and gear, our most decisive tactical advantage will always be the individual warfighter.”

He tied the initiative directly to long-term force readiness, noting that testosterone levels naturally decline with age, a fact backed by mainstream endocrinology. The screening, he said, is about catching deficiencies early rather than waiting for symptoms to degrade performance.

“This initiative, it’s not about artificial enhancement; it’s about restoring and optimizing your natural capabilities, protecting your longevity, ensuring you have the biological foundation required to sustain and fight.”

That distinction matters. Testosterone replacement therapy for clinically diagnosed deficiency is a standard medical treatment prescribed by civilian physicians every day. Hegseth positioned the military program as extending the same standard of care to troops who might otherwise go undiagnosed.
